The inspirational story of 12-year-old Jack as he waits for a new heart from a donor.\n\nJack has lived with congenital heart disease and has been in and out of hospital all his life. Now he is suffering breathing problems and can't even walk upstairs. His doctors have told him he urgently needs a new heart and he is taken back into hospital to wait for his transplant. Jack is a huge Liverpool fan and his dream is to play football and run around the pitch like his friends. \n\nThis is the biggest year of Jack's life and the documentary follows Jack’s experience on the transplant list as the children’s heart team at the Freeman heart hospital in Newcastle search for a match. \n\nJack, his mum, sister and family move into the hospital as the long search for a suitable donor begins. He talks about the highs and lows of being on the transplant list and the challenge of not knowing if or when his new heart will arrive. He has an incredible bond with his mum, little sister and gran, and Jack’s family play a vital part in keeping his spirits up alongside the doctors and nurses.\n\nAfter six months in hospital, with two false calls, eventually Jack gets the amazing news that a suitable heart has been found and his transplant can go ahead.\n\nWe follow Jack through his hours of surgery and his tough recovery process as he embraces his second chance at life. His gratitude to the unknown donor who gave him his new heart knows no bounds. All Jack wants is to do now is return home with his family, go back to school and play football with his friends.
